Christian Community Health Center (CCHC) provides quality
health care and related services to the community.
CCHC operates eight health and social service locations in underserved, low-income neighborhoods on the south and west sides, and in the south suburbs of Chicago. CCHC was founded
in 1991, is a Federally Qualified Health Center, and supportive housing and social service provider.
Mission
The mission of Christian Community Health Center is to provide high quality primary health care and related services to the community, regardless of the ability to pay.To provide services in a manner that demonstrates in word and deed the love of Jesus Christ.
Vision
The vision of Christian Community Health Center is to be a
faith-based community organization, a leader providing a range
of comprehensive medical and social services using a holistic approach, and to address the physical, spiritual, social and economic needs of the community.
We will be a prototype and mentor to other Christian organizations who desire to impact their communities as a demonstration of God’s love and compassion.

Our Organization
• CCHC provides just over 50,000 medical encounters to over 15,000 patients who visit one of our four clinics and mobile health van each year
• CCHC's Board of Directors is composed over 51% patients
and community members to represent community needs
• CCHC has a current operating budget of $15 million
Our Patients
• CCHC serves patients throughout the city of Chicago,
west and southern suburbs
• Seventy-three percent of our patients are uninsured
or Medicaid recipients

• CCHC employs over 250 employees, including Board
Certified or Board Eligible medical providers
• CCHC employs leaders in issues impacting our service
community, including HIV/AIDS prevention, sickle cell,
social justice, and supportive housing
